We make it simpler by slowing down the pace, filling the gaps between your medical appointments, and problem-solving with you in real time.
We specialize in helping adults and children who are new to type I diabetes.
Continuous glucose monitors can be helpful but they can also be overwhelming. Get help with adhesive issues, connectivity solutions, uploading data to your doctor's office, managing the many beeps and disruptions, reordering, failed sensors, and more.
A diagnosis of T1D affects the entire family and the dynamics change quickly. We'll support you as you navigate the various members of your family and ensure that everyone has what they need to feel knowledgeable, cared for, and important.
Type I diabetes comes with its own language and it can be confusing! We'll help you review what you've been taught so far, expand on your knowledge, and teach other people so they are also in the loop (and so that you don't have to keep repeating yourself to every friend, coach, and teacher).
When a child is diagnosed with T1D there is a lot to consider regarding their activities and school schedule. Post-diagnosis support is often needed to bring together the parents, the child, and the school/camp/team so that everyone is on the same page about diabetes care and expectations.
It is common for there to be a mismatch in expectations regarding diabetes management. Generally this comes from people who mean well (spouses, doctors, parents, friends) and sometimes it is actually the person with diabetes who has an unrealistic and unachievable expectation of their own care. Whatever the case, we'll talk it through and find a solution.
We'll talk about your diabetes (or your child's) and the unique challenges you are facing. All sessions are conducted over phone or Zoom.
Whether you need us all the time or just when you're feeling overwhelmed, we're here for you.
We'll personally work with you one-on-one to problem-solve in real time. You'll receive guidance, support, and practical tips and tricks. As your life with diabetes evolves, so will we.
We are real people living busy, healthy lives with type I diabetes. We understand that the concepts taught in the hospital or clinic can be tricky to implement once your real life is playing out, and we have some great tips and tricks to share from living with T1D. Plus, we have worked with thousands of children, teenagers, families, and adults who all experience diabetes differently; we know how to help you.
A diagnosis of type I diabetes impacts every aspect of your life. Since everyone's life looks different, the care and support you receive should reflect your own pace and learning style, and should involve your support networks such as family and friends. Unlike most doctor's appointments, we can tailor your support to go at your speed, involve as many or few people as you'd like, and meet you where you're at on your diabetes journey.
We do not replace your medical team, we add to it. We offer help beyond the doctor's office so that you can get advice and support in real time. That being said, we are not healthcare professionals and we want you to maintain a positive relationship with your medical team. We can help foster that relationship and add to your confidence and knowledge so that you can make the most out of your doctor's visits.
Diabetes Support Partners does not replace your medical team. Medical decisions such as insulin dosages, prescriptions, and changes to your diabetes management plan must be done by a licensed professional.
